The mail order pharmacy will deliver a 90-day supply of your medicine to your home or office with no extra shipping cost. For additional information or assistance with signing up for mail-order pharmacy, please call the CVS Mail Order Customer Support line at 1-800-552-8159.
Please tell your doctor if you want to use mail order, your prescriptions may be sent directly to mail order electronically. You may also print the form linked below and take it to your provider’s office.
