MedStar Family Choice cares about you and we want to be sure that you are getting the care that you need. We understand that some medical conditions make it hard for you to manage on your own. The nurses and social workers in our Case Management department are here to help you. We offer two types of Case Management services. These are Complex Case Management and Disease Management. These services are provided by phone.
Complex Case Management
Complex Case Management is a service MedStar Family Choice provides when you have complicated medical conditions. This service is provided by Case Managers who are nurses, and social workers. They work closely with you and your doctors. They will talk with you by phone to help arrange doctor visits, medical equipment, therapy, education and other services you may need. Complex case management is available if you have the following conditions:
- Spinal injury
- Serious trauma
- Multiple chronic health conditions
- Special healthcare needs
- COPD with one inpatient admission or 1 ER visit within 6 months for COPD
Disease Management is a service MedStar Family Choice provides when you need help managing a chronic disease. This service is provided by Case Managers who are nurses and social workers. They work closely with you and your doctors. They will talk with you by phone to help you better understand your disease. They can also help you get the testing and medical care that you need. Disease Management is available if you need help with:
- Asthma and other breathing problems
- High-risk Pregnancy
- Pain Management
- Substance Use
- Wound care
You do not have to enroll in these services. You may be enrolled if we see that you have certain conditions or medical needs. Membership in Complex Case Management or Disease Management is voluntary.
